The study recommended the following: 1. Action plan must be started for patient exit from the beginning of entering the hospital until he had got out. 2. Support head nursing staff in each hospital on the use of exit plan for patients because the increase in information given to the patient will be reflected on their education and application of information, and thus prevents the occurrence of complications for the patient and prevent patient entry repeat to the hospital.
