Abstract: The periapical region and its pathologic dynamics has been an area of much debate over the past few decades. It is extremely important for the clinician to be familiar with the dynamics of the varying microbial infections responsible for pulpal and periapical diseases as this is the basis of root canal therapy. A deep understanding and perception of this concept will in better treatment decisions and overall success in dealing with endodontic infections as well as persistent infections and failures. This paper will cover various periapical lesions and its pathological dynamics as well as the periapical Extension of Pulpal Inflammation. The Incidence of antibody type associated with each periapical lesion will be discussed and presented.
